A1. Death Benefits are payable only to the Principal Member insured, deceased Principal Member insured’s dependants, Nominees or Estate. Should an employer advance funds or directly pay for funeral costs, the employer may be reimbursed, providing FMI have signed permission from the Principal member insured, his/her beneficiaries, family or Executor - where applicable.

A2. Neither a Principal Member insured nor the Policyholder shall be entitled under any circumstances whatsoever to cede or assign any benefits under FMI’s policies.

A3. This is a two-page document completed by the under-taker or hospital, which confirms the existence of an individual and was enforced by Legislation in order to curb death fraud.

A4. In the instance where a child is stillborn or too young to have been issued with a Birth Certificate (Identification number), FMI require the BI-1663 form to verify the child’s date of birth and date of death, as well as the child’s existence. Alternatively, the family can submit a copy of the child’s clinic card, the mother’s Antenatal records or a letter from the hospital confirming the child’s existence.

In the instance where the Principal Member insured has passed away, FMI require the BI-1663 form as proof of existence. This form also provides medical information, vital in combating death claim fraud and fraudulent syndicates.

A5. An Abridged death certificate is a hand-written certificate, issued by the Home Affairs, when a deceased individual was not yet or had never been registered with the South African population registry.

A6. A Computerized death certificate is a computerized certificate, issued by the Home Affairs, when a deceased individual had already been registered with the South African population registry.

A7. This is an official document, issued by the Master Supreme Court, where an individual is duly appointed by the court to take legal responsibility for the lawful distribution of a deceased person’s assets. This document is required in the absence of a Nomination of Beneficiary or Will.
